Refer a Patient

Hospice of the Western Reserve is a nonprofit, community-supported agency that provides comprehensive palliative care and hospice services to individuals who have been diagnosed with a chronic or serious illness. We care for patients with any diagnosis, at any age (including pediatric and perinatal patients) regardless of ability to pay.


Is it Time to Make a Referral?

The following questions may help determine whether a patient needs our help. Contact Hospice of the Western Reserve's Referral Team if your patient:

  • May not survive within the year
  • Is seriously ill and pain management is needed
  • Has more than one chronic condition
  • Has been admitted to the ER/hospital several times within six months
  • Is facing difficult conversations regarding end of life decisions

If you are supporting the caregiver, the following are questions you can ask that may help determine if they and their loved one needs the assistance of Hospice of the Western Reserve:

  • How many hours do you spend caregiving?
  • Is the amount of care you're providing overwhelming?
  • Are you getting help from friends and family members?
  • Have you been ill recently?
  • Are you experiencing sleep issues?
  • Are you irritable?
  • Is the situation taking a toll on your family life or job?
  • Is it causing a financial strain?

We Can Help

Speak with the referral team by contacting us seven days a week, 24 hours a day. Any first visit and admission can be made the first day.
