Our Services

For over 40 years, Hospice of the Western Reserve has provided palliative and end-of-life care, caregiver support, and bereavement services throughout Northern Ohio.

Our services have grown to meet the requirements of a diverse 13 county area. As your needs change, our staff and services respond with customized care.


 

Our comprehensive and compassionate services and programs include:

  • Palliative care through our Western Reserve Navigator program for those who need help managing the symptoms and pain of a serious illness or multiple advanced health conditions
  • Dedicated triage and on-call team to meet unexpected patient and family needs after hours, weekends and holidays.
  • Home delivery of hospice-related medications. We deliver the equipment, the supplies and your medications to make you and your loved ones lives as easy as possible.
  • Specialized and complex care delivered for specific illnesses such as COPD, heart failure, memory impairment illnesses and renal disease.
  • Home visits by physicians and nurse practitioners. We are happy to work in partnership with your medical team, a practice we have been doing for over 40 years.
  • Our Moments to Remember Program, focused on enhancing the quality of life for hospice patients and their families by fulfilling wishes.
  • Pediatric palliative and hospice care.
  • Specialized care for Veterans through our Peaceful and Proud program.
  • Caregiver educationresources and respite care.
  • Pet Therapy Program for patients and their loved ones.
  • Art and music for patients and families.
  • Children's summer camps and art workshops for the bereaved – including two Bereavement Centers – in Cleveland and Medina.
