Location: Cleveland, OH

This is a Mon-Friday 9a-530p and every other Saturday. This position is based out of a West side Hospital.

General Responsibility: Provides an overview of hospice services within the home, hospital, or alternative home setting and facilitates the admissions process when appropriate. Coordinates implementation of an initial plan of care based on immediate needs of the patient/family unit.

Essential Functions:

· Responsible to practice in accordance with HWR clinical standards of practice, policies and procedures

· Addresses Advance Care Planning with initiation of conversations as related to end of life care and related issues

· Assess physical, psychosocial, and spiritual needs of the patient and family/caregivers

· Monitor and evaluate pain and symptoms and provide information and assistance to the patient and family to manage those problems

· Serves as liaison to the patient’s physician(s) when needed; provide information to the physician based on knowledge relating to the patient’s needs

· Develop and implement an initial plan of care

· Provide emotional support to the patient and family

· Refer patients and/or family members to other services when appropriate and follow-up as necessary

· Provide instruction (health teaching) to the patient and family

· Maintain and promptly complete all documentation, plans and reports related to admission and subsequent daily visits as related to the liaison setting

· Participate in care conferences and bereavement follow-up as appropriate

· Participate in chart review, quality improvement activities and other meetings as assigned

· Participate in discharge planning as appropriate, in collaboration with other team members and facility staff

Requirements:

Education/Experience

· Licensed RN in State of Ohio

· BSN preferred

· Experience preferred in pain and symptom management; oncology; public health; or medical/surgical background

Knowledge/Skills/Abilities

· Must have valid driver’s license, car available and insurance coverage as required in Ohio

· Computer experience helpful
