Liaison Social Worker

Location: Cleveland, OH

Provide medical, psychosocial treatment and care services to terminally ill persons and their families in collaboration with the liaison and transdisciplinary team in a variety of hospital settings.

Requirements: Masters prepared social worker; experience in counseling or related work with individual families or groups; knowledge of community resources; experience preferred in hospice, home care and/or hospital.
