Patient Family Assistant

PATIENT FAMILY ASSISTANT – AT HOME SUPPORT

Location: Cleveland, OH

This is a part-time position. Under the general supervision of a registered nurse, provides personal care in accordance with the patient’s plan of care. Works with transdisciplinary team members as needed.

Requirements: Must be a state tested nursing assistant in good standing with at least one year’s experience, and have completed a certified Home Health Aide Training Program including 16 hours classroom experience and 75 hours clinical experience; must have and maintain a valid driver’s license, good driving record and auto accident insurance at a level acceptable to Hospice of the Western Reserve.
