SOCIAL WORKER

Location: Cleveland, OH; 

Provide medical social work, counseling services, community referrals and caregiver support to chronically ill persons/families within the Western Reserve Navigator Program.

Requirements: Master’s degree; licensed in the state of Ohio; two years’ experience in counseling or related work with individuals, families and groups; experience preferred in hospice, medical facilities or nursing homes.
