Weekend Liaison Social Worker

Location: Cleveland, OH

General Responsibility:  Provide medical, psychosocial treatment and care services to terminally ill persons and their families in collaboration with the liaison and transdisciplinary team in a variety of hospital settings.

Essential Functions:

  • Assist in hospital openings, as necessary and provide support to patients and families during remainder of hospital stay
  • Complete a psychosocial assessment when need indicated by team
  • Oversee and follow patients/families opened and remaining in the hospital until time of discharge.  Coordinate plan of care with the liaison team nurse.
  • Participate in the discharge planning process and coordinate care with hospital staff.  Assist patients/families in planning for safe and appropriate         discharge.
  • Assist team with coordination of the Plan of Care which includes all disciplines including volunteer staff
  • Maintain, establish and enhance referral relationships within the hospital
  • Gather information regarding patients referred to hospice and report information to the referral office
  • Participate in Hospice of the Western Reserve liaison team conferences weekly.   May participate in hospital team conferences and/or family meetings
  • Serve as a clinical resource to hospital personnel
  • Communicate patient information to hospice team receiving the patient at time of discharge from the hospital
  • Communicate with the hospital social worker regarding change of status of the patient after discharge, i.e. re-admission to hospital, hold, death
  • If patient has been involved with home care, refer to transition guidelines
  • Complete annual competency requirements
  • Make referrals to appropriate community agencies and maintain updated knowledge of community resources
Education/Experience
  • Masters prepared social worker
  • Experience in counseling or related work with individual families or group
Knowledge/Skills/Abilities
  • Knowledge of community resources
  • Experience preferred in hospice, home care and hospital
