Admission Criteria

Hospice of the Western Reserve is a community-based non-profit organization providing comprehensive hospice services for those individuals with a serious illness.

Several clinical studies have demonstrated the benefit hospice and palliative care can have for patients diagnosed with dementia, cardiopulmonary disease, kidney disease, cancer and other terminal diagnoses. 

We are able to provide services to patients with any diagnosis, at any age, including our pediatric and perinatal programs. Through our Western Reserve Navigator program, we partner with patients to manage their pain and other symptoms for those whose diagnosis is chronic and not yet ready for hospice care.
