Avon City Leaders Attend Dementia Friends Educational Program


Sally Tomko, RN BSN, Provider Relations Manager at Hospice of the Western Reserve, recently led a one-hour Dementia Friends informational session at the City of Avon Administration Offices, 36080 Detroit Rd. Tomko is a Certified Dementia Practitioner and one of several trained “Dementia Friends Champions” at HWR who provide the free community educational sessions.

Hospice of the Western Reserve’s Commitment to Veterans Continues “Level 5” National Recognition


Hospice of the Western Reserve (HWR) is thrilled to announce it has been renewed as a “Level 5 Partner” with We Honor Veterans.  A program of the National Hospice and Palliative Care Organization, WHV addresses the growing need for veteran-centric hospice and palliative care. Over the past 18 months, HWR has cared for more than 2,085 veterans through its Peaceful and Proud program.
