After the initial call, we will schedule a visit to your home or care setting as soon as possible. The team member will complete a thorough assessment of your loved one’s symptoms, medications and overall condition. During this time, they will also explain hospice services and review what to expect.
Creating a Personalized Care Plan
Once admitted, the hospice team creates a care plan tailored to your loved one’s needs. This plan includes medical, emotional and spiritual support for both the patient and family. A nurse will manage medications and coordinate with your doctor. Social workers, aides, and volunteers will provide added support to make daily life easier.
Ongoing Communication
