Western Reserve Navigator is a non-hospice palliative care program that focuses on helping patients manage their symptoms, supporting them throughout their illness and assisting the family in finding additional, community resources. Speak with the referral team by contacting us seven days a week, 24 hours a day.
You may want to consider palliative care if you:
Have been newly diagnosed with an advanced illness or there has been a change in your clinical course of care
Want to continue treatments to cure your illness while maintaining your quality of life
Have frequent hospitalizations or trips to the emergency room for your advanced illness (2 or more visits within 6 months)
Need help managing pain and symptoms associated with your illness
Struggle to visit your doctor or miss important appointments
Need help understanding your treatment options and/or completing advance care planning
Our team consists of advanced practice registered nurses, social workers, registered nurses, and trained volunteers. Additionally, we provide dedicated after-hours support through our Care Solutions Center.
Speak with the referral team by contacting us 24 hours a day, seven days a week. We will do our best to schedule a visit on the same day it is needed.
