Each of our physicians and 89% of our clinical leadership are hospice and palliative care certified. By emphasizing continuing education and staff certification, we assure our patients and their families that they receive care from knowledgeable and exceptionally skilled professionals. Hospice of the Western Reserve is accredited by independent, nonprofit organizations, including the Accreditation Commission for Health Care (ACHC) and Community Health Accreditation Partner (CHAP).

At Hospice of the Western Reserve, we process referrals quickly and would be happy to contact your patient to begin care. We make every effort to schedule a same-day visit when family is available, recognizing patients’ vulnerability during the transition from hospitals to post-acute care settings or the community. We offer specialty care for patients to help alleviate symptoms, improve quality of life and decrease hospitalizations. Specialized services include treatment for patients diagnosed with COPD, HIV/AIDS, and progressive memory disorders, support for veterans and a committed team for pediatric palliative and hospice care. If your patient is dealing with an advanced serious illness, palliative care might be a suitable option. We deliver palliative care through Western Reserve Navigator and our team's primary aim is to offer comfort and support to both the patient and their loved ones. Our expertly trained and skilled providers collaborate with caregivers and patients to manage physical symptoms, facilitate conversations on advanced care planning, and identify their goals, wishes and preferences. These services are delivered by advanced practice registered nurses, social workers, a registered nurse for phone support and a group of volunteers who enhance care with monthly touchpoints, companionship, or respite. We invoice the patient's insurance and accept Medicare, Medicaid and most commercial insurances; however, a copay may be required.
